The RN measures, photographs, and documents the wound — size, depth, drainage, odor, and surrounding tissue — on every visit to track healing progress.

Wound Cleaning & Debridement

Irrigation, cleansing, and removal of non-viable tissue to promote a healthy wound bed, reduce infection risk, and support healing.

Wound Dressing Application

Clinically appropriate dressings applied as ordered — silver-impregnated, moisture-retentive, antimicrobial, or compression-based depending on wound type.

Patient & Caregiver Education

Teaching you and your caregiver proper wound care technique, warning signs of infection, nutrition for healing, and when to contact your physician.

Physician Coordination

Communicating wound status directly with your doctor — updating the care plan, escalating concerns, and requesting additional orders as healing progresses.
